Is it worth getting if you already have the first one? i heard it is the same thing almost...

To put it simply, it's more or less an update.  There are new weapons, new stages and new BOSS INFECTED.  Plus the difficulty level has been raised for both survivors and boss infected alike

theres also melee weapons now, basically its there to make you go insane when playing with non-experts on expert difficulty, dont even think about doing realism mode unless you got 3 good friends with voice chat

Yes, in the Original, if survivors die, you can bring them back to life after passing certain points in the game, however in Number 2, there is a mode called "REALISM" in which if a survivor dies, they are gone for good and you must finish the campaign without them.
